Modernizing Medicaid NEMT Verification

Direct Appointment was built to solve one of healthcare's most overlooked inefficiencies — the manual, error-prone process of verifying Medicaid transportation appointments between providers and brokers.

Who We Are

Our Mission: Smarter Data for Better Patient Access

Direct Appointment was founded with a single purpose: to modernize the Medicaid Non-Emergency Medical Transportation (NEMT) ecosystem. We saw a broken process — thousands of daily phone calls, manual verifications, ghost trips, and compliance gaps — and built a smarter way. Our platform creates a seamless, automated data bridge between Electronic Health Records (EHR) and Transportation Brokers, delivering verified appointment data in real time at zero cost to brokers and providers.

"Smart Data. Seamless Access. Better Care."

— The Direct Appointment promise to every broker, provider, and patient we serve.

Rachel Scholler
Rachel SchollerCo-Founder | Medicaid Transportation Provider Operations

Rachel Scholler spent 17 years building and operating a Wisconsin-based Medicaid transportation company, growing it into a highly successful operation before successfully exiting the business. Throughout those years, she gained firsthand experience managing scheduling, dispatching, broker communication, compliance, trip verification, staffing, and every aspect of day-to-day transportation operations. After nearly two decades working within the system, Rachel recognized the need for technology designed around real provider workflows—not theoretical processes. Her operational experience became the foundation for Direct Appointment's provider-focused design, ensuring the platform delivers practical solutions that improve communication, reduce administrative burden, and simplify transportation coordination.

Looking Ahead

Our Vision

We believe Medicaid transportation can be simpler, more connected, and more efficient for everyone involved.

By connecting healthcare providers, transportation brokers, and transportation providers through one intelligent platform, Direct Appointment helps reduce administrative burden, automate appointment verification, improve communication, and enhance patient access to care.

Our goal is not simply to improve transportation scheduling — it is to modernize the way healthcare organizations, brokers, and transportation providers work together.

Direct Appointment wasn't built from theory. It was built from decades of firsthand experience on both sides of Medicaid transportation.

The Challenge

The Medicaid NEMT Problem We Solve

After decades working within Medicaid transportation, we repeatedly witnessed the same costly challenges: manual scheduling, disconnected systems, appointment verification delays, preventable fraud, and fragmented communication between healthcare providers, transportation brokers, and transportation providers. Rather than treating the symptoms, we set out to solve the underlying problem. Direct Appointment was created to connect healthcare providers, Medicaid transportation brokers, and transportation providers through one intelligent platform that automates scheduling, verifies appointments in real time, reduces administrative burden, and improves patient access to care. Our mission is simple: modernize Medicaid transportation through technology that works for every stakeholder involved

Our Principles

The Principles Behind Our Platform

Integrity in Every Record

We build systems that create honest, verifiable records — protecting Medicaid providers, transportation brokers, and patients from fraud, disputes, and compliance failures.

Innovation Through Integration

We leverage secure API connections with leading EHR platforms to automate what was once entirely manual — turning days of phone calls into seconds of data exchange.

Access for Every Patient

We believe every Medicaid patient deserves reliable transportation to their medical appointments — and every broker deserves accurate, real-time data to make that happen without friction.

Accountability at Every Step

Our 100% digital audit trail ensures state Medicaid compliance, eliminates billing disputes between providers and brokers, and creates a clear chain of verification for every trip.
